"A whole system working to improve health, social wellbeing and provide more
efficient health and social care services"
The Community Care Section of the Community Services Department of the Moray Council is integrated with the National Health Services to form the Moray Community Health & Social Care Partnership. The Partnership brings together all NHS services in Moray: Primary Care, Acute Services/Dr Grays Hospital, Health Improvement, with Social Work services for all Community Care groups.
These include specialist and generic services for older people, mental health (including old age psychiatry and addictions) learning disability, physical and sensory disability, and carers.
A single manager with a professional social work, occupational therapy or nursing background manages these integrated services on behalf of the Partnership.
The Partnership also takes responsibility for Community Planning, to deliver on the theme of Achieving a Healthy and Caring Community. The Partnerships objectives therefore embrace the Councils corporate priorities in conjunction with NHS and Community Planning priorities.
